The Challenge

Our client, a leading U.S. healthcare company, aimed to develop a new product focused on providing patients with self-managed care plans that would enable:

  • Clinicians choose to send invitations to patients.

  • Patients can log in and follow predefined care paths while reporting their outcomes.

  • Clinicians can modify the content of the plan and closely monitor the program's success by receiving periodical updates and alerts.

The Solution

We helped our client to create a platform that empowers patients to take control of their own treatment, fostering greater independence and engagement.

Clinician Portal

We created a robust portal for clinicians to efficiently manage patient care.

Key features include:

  • Patient Management: Clinicians can create patient profiles and assign tailored care plans.

  • Flexible Care Plans: Multiple plans can be assigned to a single patient to address various areas of improvement. Each plan is meticulously curated by the medical board but allows customization—clinicians can substitute exercises with alternatives from a predefined library.

  • Comprehensive Content: From educational lessons delivered through videos and text-based materials to exercises accompanied by video demonstrations to ensure proper guidance.

Patient Progress Dashboard

Clinicians are equipped with a dynamic dashboard that highlights patient outcomes, which are either auto-generated or reported by the patient through surveys.

This enables proactive decision-making:

  1. Red Flag Alerts: Immediate notifications if a patient’s progress stalls or if risks are detected during exercises.

  2. Adaptive Plan Management: Move patients to the next level upon improvement or pause/cancel plans if there is a risk of falling or increased pain.

  3. RTM Billing Support: A recent addition tracks time spent analyzing patient data and interactions, simplifying the generation of Remote Therapeutic Monitoring (RTM) billing reports.

Patient Platform

For patients, we developed an intuitive web application that enhances engagement and ensures ease of use.

  1. Daily Activity Overview: A modern home screen with activity cards outlining the day’s tasks.

  2. Gamification for Engagement: Encouraging participation through interactive elements and rewards.

  3. AI-Powered Exercise Feedback: New functionality records the patient’s exercises via video while AI analyzes movements in real time to detect risks (e.g., potential falls) and immediately notifies clinicians.

  4. Progress Tracking and Feedback:

  • Patients complete periodic surveys based on industry standards (e.g., MSK, incontinence, falls).

  • Forms are presented at key milestones to confirm progression through the program.

  • A visual timeline displays progress based on patient-generated feedback.

Key Outcomes

  • Pain: Reported Improvement 71%

  • Function: Reported Improvement 66%

  • Social activities: Reported Improvement 61%

  • CSAT 92%: Invites sent per month 216

  • Clinician using the app this month: #64 (+42% from previous)
